You call or use the contact form. We take a few minutes to understand the referral question and confirm the evaluation is the right fit.

Tailored, standardized assessment
Each assessment is tailored to the individual, using evidence-based, standardized tools appropriate to the referral question. In-person and virtual options are available.

Clear, understandable report
You receive a clear, understandable written report designed to guide next steps. Please contact the office to discuss scheduling and timelines for your specific evaluation.
Fees & Payment
Self-pay and some insurance panels accepted.
Individual self-pay rate and sliding scale offered. Please contact the office to verify benefits and discuss fees.
